Trauma & Emotional Well-Being Self-Check

Trauma & Emotional Well-Being Self-Check

A closer look at trauma-related patterns... intrusive memories, avoidance, negative shifts in mood and beliefs, and hyperarousal... alongside anxiety, mood, and relationship patterns.

Before You Begin

A Little About me.... I’m Rachel Farnsworth, founder of Wellness With Rachel, and I’ve spent many years helping people understand what may be happening beneath the surface of the symptoms, emotions and patterns they experience. So very often, difficult past experiences continue to shape the way we think, feel, sleep, relate to others, or respond to stress, often without us realizing it. This self-check is designed to help you notice those patterns, with a particular focus on trauma-related responses.

Important: This questionnaire is for educational and self-reflection purposes. It is not a medical diagnosis and cannot determine whether you have PTSD, depression, an anxiety disorder, or another mental-health condition.

For each statement, select the answer that best describes how often you have experienced it during the past month.

  • 0 → Not at all
  • 1 → Occasionally
  • 2 → Sometimes
  • 3 → Often
  • 4 → Very often / almost always

One Important Safety Question

During the past month, have you had thoughts that you would be better off dead, thoughts of hurting yourself, or thoughts of suicide?

Your safety is important.

Because you indicated that you have experienced thoughts of death or self-harm, this online self-check should not be used as your only source of support.

Please consider contacting a qualified mental-health professional or an appropriate emergency/crisis service in your location, particularly if you feel you may act on these thoughts or cannot keep yourself safe.

If you are in the U.S.: Call or text 988 (Suicide & Crisis Lifeline), available 24/7, or text HOME to 741741 (Crisis Text Line). If you are outside the U.S., please look up your local crisis line.

If you are in immediate danger, contact your local emergency services or go to the nearest emergency department.

What Your Result May Mean

My approach is different because I don’t believe your symptoms are something to simply fight against.

I help people become curious about those patterns and begin to understand what their mind and body may be communicating.

This self-check is simply a starting point. It’s an opportunity to pause, become curious, and notice what has been showing up in your life, especially around trauma-related patterns, because so often the most powerful question is “What is this pattern trying to show me?” It’s simply information, and information gives you somewhere to begin.
