A Little About me.... I’m Rachel Farnsworth, founder of Wellness With Rachel, and I’ve spent many years helping people understand what may be happening beneath the surface of the symptoms, emotions and patterns they experience. So very often emotional experiences continue to affect the way we think, feel, sleep, relate to others, or respond to stressful situations. This self-check is designed to help you notice those patterns.

Important: This questionnaire is for educational and self-reflection purposes. It is not a medical diagnosis and cannot determine whether you have PTSD, depression, an anxiety disorder, or another mental-health condition.
